Post Bankart Repair: subject complaining of right neck tightness
A 27-year-old lady complains of right shoulder end of range weakness and right neck tightness post Bankart repair four and half months ago.
She feels that her shoulder range has improved to full range over the last four months but feels that her shoulder ROM still feels tight at end range. She also finds her neck stiffness is disturbing as she experience the tightness as the day progresses. She finds no relief of the neck tightness over the last few months despite regular stretching. She does feel well in the morning.
O/E: Cx rom and should rom is full, except right shoulder external rotation is limited to 70 because of shoulder tightness.
Accessory glides: GH joint mobility nomal. Right C1-3 and C4-6 is stiffness in both AP- PA- direction.
Muscular system: noted weakness of right shoulder muscles, especially so in outer range. (generally 3-4/5) There is also poor scapulo-thoracic movement with slight winging of R scapula.
noted upper trapezius and scalenus muscles are tight. Mild tightness and tenderness palpated at R paravert muscles.
Neural system: ULTT 2a appeared positive with the rest -ve. Nerve trunk palp appeared non-significant.
PNF is non significant.
Any ideas to improve her neck stiffness and shoulder control?
